If you work with people that might need support to try something new, get active or speak to someone about benefits, housing or money worries, you can refer them to Connect Well Hull.
Connect Well Hull is a friendly, free and confidential service for local people who live in Hull or are registered with a Hull GP.
We help people to manage their health better and improve their wellbeing by providing advice and supporting them to access local community groups, activity and services. This is sometimes called ‘social prescribing’ because the service looks at finding different, non-medical ways of supporting people to better health or improved independence. Similar schemes running in other parts of the country have been shown to help improve mental health, reduce social isolation and loneliness, help improve physical health and bring people together in the community.
Everyone is different, so our Wellbeing Co-ordinators will help people find out what’s right for them and support them to access the right group or organisation. Examples could include support to access an exercise class, a gardening group, a lunch club or to join with others to manage a long-term health condition such as diabetes.
Our Welfare Advisers will offer advice on a wide range of issues, from money to family problems, to benefits, housing and employment.

The service is funded by Hull Clinical Commissioning Group and Hull City Council, and delivered by Hull & East Riding Citizens Advice and North Bank Forum, working in partnership.
